Jamie makes some great points but I would also add that defining the problem statement effectively is critical. I wrote about this here:
http://leanjourneytruenorth.blogspot.com/2009/11/defining-problem-statement.html

Tim McMahon
A Lean Journey Blog
http://leanjourneytruenorth.blogspot.comTim McMahonhttps://www.blogger.com/profile/07780727768370988177noreply@blogger.comtag:blogger.com,1999:blog-61294669138147395.post-43735989685980688402009-12-18T10:38:13.440-06:002009-12-18T10:38:13.440-06:00Great post. Most of them work just fine. The problem is with the understanding of their use and the behaviors people exhibit when thinking about problems.

I think people get themselves into trouble before they even pick up a tool in that they don't have high agreement about what a problem even is and which one they should solve. I'll survey 5 members of the same team and 4 of them will give a different list of priority problems and the 5th says "we don't have an problems."

We need to improve how people write problem statements, which I wrote about in my column here: http://www.assemblymag.com/CDA/Articles/Column/BNP_GUID_9-5-2006_A_10000000000000616075

and we also need to teach people the difference between solving spot surface problems instead of systems problems. You talk about that above and I think it is a key issue. I talk about it here: http://jamieflinchbaugh.com/2009/12/solve-your-own-darn-problems/Jamie Flinchbaughhttps://www.blogger.com/profile/16714555426822621398noreply@blogger.com
